Easy to get my insurance even with medical problems but the price difference in the space of 3 months was over £30 more for exactly the same cover

I've looked at the last couple of policies, and it seems your previous cover was for a trip to Portugal - and this time you're travelling to Spain. We haven't increased pricing on this product, but the change in country (due to associated risk and medical expenses in these areas) explains the difference in price.
